PROPOSAL GUIDELINES**
(**The Izumi Foundation will only consider those organizations that are invited to submit proposals following a letter of inquiry; unsolicited proposals will not be considered).
The Foundation requests that a proposal contain the following information:
- A brief explanation of the history and purpose of the organization, including experience with past projects, a description of both the population and the geographic area that the organization serves.
- A detailed explanation of the problem or need that is being addressed by the project, and a justification for why the proposed project is important.
- A brief overview of the project: Include geographic scope; population served; timeline; and a brief summary of the main activities. (1 paragraph)
- A list of the main health indicators for the country where the project will take place (including infant mortality rate, maternal mortality rate, child mortality rate, life expectancy at birth, as well as any other relevant disease indicators). Please be sure to include your sources.
- An explanation of both the overall project goal, as well as the specific objectives of the project.
- A detailed list and timeline of the project activities that will be supported to reach stated objectives.
- An explanation of the ways in which objectives and activities are in line with priorities identified by government or international agencies for that region or population group. If the project addresses a problem not designated as a priority, please provide justification for its importance.
- An explanation of the ways in which the project activities take advantage of or coordinate with existing in-country services, the existing health system, government programs, or other services provided by local organizations.
- A description of the methods that will be used to evaluate outcomes and a list of proposed indicators. (If the project does not include any type of formal or informal evaluation or assessment, please state so here and move onto the next question).
- A summary of the personnel who will implement the project and their qualifications to do so.
- An explanation of the sustainability plan for the project. Include intended future funding and methods of support.
- A budget for the period of the project. (Note: The Foundation excludes rent, equipment leases, utilities, insurance and membership dues as budget items because these costs are considered to be part of an organization’s overall operating budget).
a. Overall Budget: (A sample budget format is available upon request).
b. Budget Narrative: A narrative explanation of the project costs should be provided. Costs within each budget category must be itemized. If travel money is requested, please detail the cost of each trip.
c. Additional funding sources: The budget should detail any in-kind contributions or other funding that will support the project. The “total other funding” line item (on the sample budget) represents the total amount of in-kind support and other funding committed.
d. Financial Proof: A copy of the organization’s operating budget and most recent audited financial statement is required.
NOTE TO APPLICANTS:
- The Foundation does not require any additional materials, such as videos, brochures or photos, as part of the proposal. If any additional information or documentation is needed, the Foundation will contact the applicant organization.
- The Chief Operating Officer of the applicant organization must sign the proposal. The Foundation requires a hard copy of the proposal and will not accept an electronically submitted proposal.
- Grant recipients are required to file both interim and final financial and evaluation reports, and agree to the terms and conditions of the grant award and grant payment schedule.
